We are required to deliver HF Training within the organisation – but how do we know whether it is effective or not?

Consider to measure anything we need to have a reference point or points which we can then use too gauge how effective our training program is.

So where are the measures?

i/ How are we assessing the contents of the training does it reflect organisational and SMS issues?;

ii/ Are we measuring the engagement of employees – this can be done through either a survey or multi choice exam for example.

Now consider some of the challenges

1/ Do we identify all our exposures in the way that best supports the organisation, put it another way if people do not report then it is not going to be possible to act on the event or potential event;

We have two aspects related to the identification of issues:

a) Events that have happened.

b) Events which did not happen but were identified as an exposure and possibly were not reported.

2/ How effective are we within the organisation at identifying the “cause and effect” relationship;

We should always be able to make a direct connection between the various root causes and the negative or bad outcomes. Note the importance of showing in every case the why ?

All errors which are caused by or directly related to process and procedures must also have a preceding cause.

We need to understand again the root cause of the procedural violation so that we can manage by seeking to eliminate the precursors. (At this point we may also consider the difference between organisational optimising violations and personal optimising violations.)

If we are going to add value to the system we must ensure that we understand why we have experienced the event or potential event so that we can address the root exposure.

3/ Avoid over simplistic explanations which are too “watered down” to be useful. Ensure a person reading your information will have a clear understanding.(For example “complacency is not an acceptable statement” as it is of no value without substantiation) Do not use inflammatory statements which may also be counterproductive;

4/ Every human error must have a preceding cause. Again we must focus on the Why until we understand the true nature of the exposure;

5/ It is a detailed understanding of the cause of the error, rather than the error itself, which leads us to develop effective prevention strategies.
