Significant changes have taken place over the last 25 years in many areas of aviation and not only in the technical quality of the product. It has taken the Aviation Industry decades to understand fully the role of the organization in aviation accidents and incidents. This understanding has led to the current environment in which the focus is turning within the organization to the specific need for a Safety Management System, not just driven by regulations but by the organisations desire to promote the most effective business practices.

It was not until the early 1990s that it was accepted following the analysis of many incidents and accidents that individuals do not operate in a vacuum, but rather within defined operational contexts. This signalled the beginning of what has become know as the “organizational era”. Within the organization, safety is increasingly viewed from a systemic perspective, in so doing to encompass a range of organizational, human and technical factors.
