9/ Lessons learned

The aim of the Aviation Maintenance Post Check Analysis and Follow up activity is to aim to drive improvements, to optimize the existing processes, and to identify the opportunities:

1/ Any issues arising – In general to deal with any issues in a consistent way

a) What is the issue?

b) Why did it occur?

c) Does it need to be referred to Quality or Safety for additional actions? – If yes generate an internal reporting document

d) Does it impact an existing behavior or procedure – if yes allocate an action against a responsible person

2/ Manpower utilization – actual versus estimation

The MPD identifies a minimum time within which the task and or check should be accomplished

What is the standard for our organization?   MPD x organization factor (note that the lower this factor is the more economical is the activity).

a) Can we identify any issues which have an impact on the effective use of our manpower – yes identify and forward to Production Planning Program Optimization Manager.

3/ Lost time summary

a) Do we have procedures to identify lost time (for what ever reason?)

b) For each lost time issue

i) What is the issue?

ii) Why did it occur?

iii) Does it need to be referred to Quality or Safety for additional actions? – If yes generate an internal reporting document

iv) Does it impact an existing behavior or procedure – if yes allocate an action against a responsible person

4/ Defects found during the maintenance check

It is normal to find defects during maintenance – this is why we do maintenance. The information however may be analyzed as part of the reliability process.

This information is also an indicator of the general and overall effectiveness of the maintenance.

A lower level of defect reporting is an indication of effective maintenance

5/ Defects deferred during the maintenance

Defects deferred during maintenance reflect on the ability to support the maintenance product, so how do we follow up to understand if there are any exposures which should be addressed?

a) What is the issue?

b) Why did it occur?

c) Does it need to be referred to Quality or Safety for additional actions? – If yes generate an internal reporting document

d) Does it impact an existing behavior or procedure – if yes allocate an action against a responsible person

6/ Logistic issues

The ideal situation is where the correct spares and material are preloaded for the maintenance check.

If for some reason we have to order on an urgent basis, it can impact the costs.

To understand and address or if possible mitigate any issues arising is a beneficial activity

a) What is the issue?

b) Why did it occur?

c) Does it need to be referred to Quality or Safety for additional actions? – If yes generate an internal reporting document

d) Does it impact an existing behavior or procedure – if yes allocate an action against a responsible person

7/ Manpower issues

Manpower allocation to a maintenance check is one of the most significant expenditures. It is important to ensure that we allocate resources effectively.

Too little manpower will be flagged up – but how to establish if we have too much manpower.

The more detailed the manpower utilization recording systems the better will be the analysis of the data.

8/ Documentation Issues

Any documentation issues can cause manpower wastage and additional costs.

d) Does it impact an existing behavior or procedure – if yes allocate an action against a responsible person

9/ Lessons learned

Have we learnt any lessons from this meeting?

If so can we develop actions – against whom?- or develop into the need to understand to a deeper level

Where there any Human factor issues during the maintenance Check?

Maintenance which is not carried out correctly may contribute to an aviation incident or accident.

For example parts which are installed incorrectly, parts which are missing, also incomplete maintenance for a variety of reasons.

Accepting that maintenance personnel may be responsible the problem is only part of the issue, as there may be related organizational factors which contribute to the outcome.

A detailed investigation may be appropriate to examine the risk and exposure which is present in the organization which is either causal or contributing to the issue in question.
