    • SMS Requirements for Operations: This includes descriptions of the procedures and processes necessary to ensure that the organization’s SMS aligns with regulatory requirements and is implemented across all levels of operations.
    • Accountability and Responsibilities: The documentation must clearly define the roles, responsibilities, and authorities of key safety personnel (such as the accountable executive and the safety manager). This also includes the description of their responsibilities for implementing and maintaining SMS processes.
    • SMS Processes and Procedures: Organizations are required to document their safety processes, including hazard identification, risk assessments, safety assurance measures, safety promotion, and continuous improvement activities
  • Challenges:
    • Capturing all relevant data across different operations and departments.
    • Ensuring that the documentation stays up-to-date with the continuous evolution of processes, especially in complex organizations.
  • Best Practices:
    • Use a centralized digital document management system to organize and maintain all SMS documentation, ensuring that updates are easily made and accessible.
    • Standardize formats and templates for safety documentation to ensure consistency and ease of understanding across different levels of the organization.

SMS Records Requirements (§ 5.103)

  • Categories of Records: The certificate holder is required to maintain various categories of records to ensure that the SMS is functioning effectively. These records must include:
    • Safety Risk Management Records: All outputs from the safety risk management process, such as risk assessments, hazard identifications, and the controls implemented to mitigate those risks. These records must be retained for as long as the controls remain relevant to the operation.
    • Safety Assurance Records: These include safety performance monitoring data, results from safety assessments, audits, and reviews of the SMS and operational processes. Records related to safety investigations, incident reports, and corrective actions taken also fall under this category and must be retained for a minimum of five years.
    • Training Records: Documentation of all training provided to personnel as required by § 5.91, especially related to their SMS roles and responsibilities. Training records must be kept for at least five years following the completion of the training.
    • Safety Communication Records: Records of all safety communication activities conducted as required by § 5.93. These records, too, must be maintained for a minimum of five years
