Understanding Human error also addresses both tolerance as well as our ability to recover. When we fail because of Human Factors do we have an option of recovery which avoids any impactful operational upset?

We WILL see Human Factor Errors and when they occur we should ensure we have the capacity to ensure effective recovery.

Safety is not just the absence of accidents, safety is our ability to understand our system and to reduce our risk exposure. It is not correct to say that we improve safety by eliminating bad things, rather we make safety better by improving our systems, processes, planning, and operations.

Our performance is influenced by working with other people, technology, and the environment.

People working together can do more collectively than any individual can do alone.

  • Individuals may be heavily dependent on the actions and behaviours of other people from another group who may be working under different constraints and goals.
  • Performance is also influenced by technology & environment, new or modified tools, even when well-designed, may result in changes to how people perform their tasks and impact roles and responsibilities.
  • Accident Investigation often identifies that a failure to follow procedures is a precursor in many incidents and accidents. Unfortunately, the answer to the question why? Is not so well understood

Humans will remain responsible for ensuring the safety of our aviation industry whilst technology continues to evolve and impact our ability to fully understand human interaction.

To ensure the best possible outcomes it is necessary to establish a solid scientific basis by which we can assess human performance implications in all domains.

It should be noted that the following principles are typically interrelated with each other and can affect and impact the overall situation in any number of ways, consider the following as a basis for understanding:

  • If you place good people into bad systems, you will undoubtedly get undesired outcomes!
  • Performance is shaped by people’s capabilities and limitations
  • We interpret situations in different ways and respond in different ways
  • We adapt to meet the demands of a complex and dynamic work environment
  • People Assess Risks and Make Trade-Offs
  • Error is normal (Even the best people make mistakes)
  • Blame fixes nothing.
  • Learning and improving is vital. Learning is deliberate.
  • Context influences behavior. Systems drive outcomes.
  • How you respond to failure matters. How leaders act and respond counts.
  • People’s performance is influenced by working with other people, technology, and the environment
