Introduction: Accountable Manager (AM)

The Accountable Manager (AM) assumes safety accountability which means that he, or she, is ultimately accountable for safety in the Company.

The AM endorses the Safety Policy; provides the human and material resources necessary for operating the SMS and achieving the safety objectives; nominates the Safety Manager, the Compliance Monitoring Manager, the Safety Review Board, and the Safety Action Group.

  • Note: the AM Chairs the Safety Review Board (SRB), and endorses the safety objectives and the Compliance Monitoring Report.

Introduction: Safety Manager (SM)

The Safety Manager acts as the focal point and is responsible for the development, administration, and maintenance of the SMS. The Safety Manager is the focal point for collecting and analyzing hazards and maintaining a register of hazards, risks, and risk controls (mitigations).

The primary functions of the Safety Manager include:

  • Facilitate hazard identification, risk analysis, and management;
  • Monitor the implementation of actions taken to mitigate risks as listed in the safety action plan;
  • Provide periodic reports on safety performance;
  • Ensure maintenance of safety management documentation;
  • Ensure that there is safety management training available and that it meets acceptable standards;
  • Provide advice on safety matters; and
  • Ensure initiation and follow-up of internal occurrence/accident investigations.

The Safety Manager is the focal point for collecting and analysing hazards and maintaining a register of hazards, risks, and risk controls (mitigations).

Depending on the size of the operator and the nature and complexity of its activities, the Safety Manager should be assisted by additional safety personnel for the performance of all safety management related tasks. State which members of staff have been designated to assist the Safety Manager, if applicable.

Regardless of the organizational set-up, the Safety Manager remains the focal point as regards the development, administration and maintenance of the SMS.

Concerning Barriers

The Following Barriers May Contribute or Present an Acceptable Protection Related to a Potential Exposure Identified During a Risk Assessment.

  • Inherent Design of Aircraft, Equipment & Infrastructure Providing Acceptable level of Protection
  • Effective Process & Procedures regarding the Maintenance & Rectification of Known Defects
  • Adequacy of Operational Support and the Existing Management of Problems Encountered
  • Awareness and System Addressing Related to any Technical Factors
  • Process & Procedures SOP’s related to Tactical planning
  • Organizational and Individual planning prior to the flight or other operational activity that supports the reduction of the causes and contributors to accidents
  • Organizational or Department Collective Knowledge of Regulations, Procedures & Processes
  • Acceptable level of competence throughout the organisation related to effective, understandable and available regulations, procedures and processes that are complied with
  • Situational awareness and action, includes human vigilance for operational threats which requires specific human intervention to ensure identification of operational hazards and effective action to prevent an accident
  • Warning systems operation and action that could prevent an accident and which are fit for purpose, functioning, operational and are complied with
  • Process, Procedures and Behaviours which enable late recovery from a potential accident situation
  • System ‘Protections’, when an event has occurred, which contribute to reducing the level of the outcome providing for a mitigation which prevents the escalation of the occurrence by intangible barriers or providence
